Infusion and Specialty Pharmacy Delivery

When receiving infusion therapy, medicine is given through a needle or catheter. Receiving infusion and therapy at home is an excellent alternative to an extended stay at a hospital or nursing home. Physician Referrals to Home Healthcare

If you need to refer a patient for home care, Atrium Health At Home is happy to help. We have a centralized referral intake system where clients are given a single contact at the time of referral.

Registered nurses are available to assist you by:

• Acting as the primary contact person for referrals that require skilled nursing, infusion or rehabilitation services
• Obtaining case management and authorization for all services as insurance requires
